Documents Required for Admissions in D-PREP

1. Completed and signed application form.
2. Go to Online Application
3. Copy of parent’s passport or Thai I.D. card.
4. Copy of child’s passport or Thai birth certificate.
5. Copy of Non-Immigrant “Ed.” visa – foreign students.
6. Copy of Thai residence registration.
7. Copy of Previous school records including report cards and all transcripts.
8. Copy of Home map if using ASB school buses for transportation.
9. 4 pictures – Each child (size 2″ X 2″).
10. 4 pictures – Each parent (size 2″ X 2″).
11. 4 pictures – Every person with permission to pick up the student (size 2” x 2”).
12. Detailed health form.
